Informed Consent and Assumption of Risk

Every participant must submit their Informed Consent and Assumption of Risk form prior to participation in any program or activity. This form must be completed following the first registration of each new gymnastics season. A link to the form will be sent to you immediately following your registration.

Medical Information & Participant Accommodations

The Club will endeavor to support all children, including those with physical, intellectual, social, emotional, and behavioral issues. Understanding the needs of children who attend our programs will help our Club ensure everyone can participate to the fullest extent of their interests and abilities. It is the responsibility of the account owner, when the participant is under the age of 18, to advise the Club of medical conditions, such as allergies, asthma, and diabetes; developmental delays; learning disorders; behavioral issues; and/or anything else that may require our staff to provide additional support and/or accommodation. The request for accommodation process will not require disclosure of any diagnoses or medical information that is not necessary for considering modification to programming. A Medical Information and Accommodation Request form will be emailed to the account owner, immediately following registration. To ensure our staff are aware of all participant needs and to ensure appropriate arrangements are in place in a timely manner, requests should be made at least two weeks prior to the participant's first scheduled activity or immediately following registration if there are less than two weeks before the first scheduled class/training.
