Liability Release:
The Parent/Legal Guardian named above certifies they have authority with the respect to custody and care of the participant and understands Beaches Reading Clinic Inc. is relying on this certification. The Participant and/or the Parent/Legal Guardian gives permission for the participant to participate in any of the following applicable Beaches Reading Clinic activities: tutoring sessions, community events, workshops, drop in programs/clinics, camps and/or camp activities. The legal guardian listed above releases Beaches Reading Clinic Inc., including directors, employees, volunteers, sub-contractors, vendors, from any and all liability from any claim, demand, action or cause of action of any kind arising from any illness, injury or death of the participant as a result directly or indirectly participating in an activity listed above with Beaches Reading Clinic Inc. This indemnity extends to and includes all costs, legal or otherwise, incurred in defending such claims, demand, action or cause of action.

Medical Release:
The Participant and/or the Parent/Legal Guardian herby certify that the Participant does not suffer from any physical, mental, or medical condition that would pose a risk to the Participant while attending or participating in any Beaches Reading Clinic Inc. activities including: tutoring sessions, community events, workshops, drop in programs/clinics, camps and/or camp activities. By submitting and signing this form, I acknowledge that participation in activities with Beaches Reading Clinic Inc. involves very little risk, however the participant could sustain illness/injuries (minor, serious, catastrophic or fatal). The Parent/Legal Guardian, on his or her own behalf and on behalf of the Participant, herby assume all responsibility for any such risk and releases Beaches Reading Clinic Inc. from any claims, demand, action or cause of action of any kind with respect to such illness/injury. The Parent/Legal Guardian authorizes Beaches Reading Clinic Inc. to provide first aid treatment in event of an injury. If no medical treatment is provided, or if the provided treatment is ineffective, Beaches Reading Clinic Inc. will not be liable in any way. The Parent/Legal Guardian shall be responsible for all costs of any such treatment and indemnifies and holds harmless, Beaches Reading Clinic Inc. or representatives in this regard.

Use of Image:
The Parent/Legal guardian acknowledges that Beaches Reading Clinic Inc. and affiliates from time to time produce video, website content, social media posting, photographs, printed material and voice recordings for promotional or commercial use. The Parent/Legal Guardian, on his or her own behalf, and on the behalf of the participant, acknowledge that the Participant may be included in such recordings as a participant or spectator with Beaches Reading Clinic Inc. and consents to the same. The Parent/Legal Guardian hereby transfer and assigns to Beaches Reading Clinic Inc., the exclusive right to make such recordings and utilize the Participant's image and/or voice for current or future promotional or commercial use.

Late Payment Fee Policy: An additional fee of $75 will be required to continue tutoring for that month. Accounts must be in “good standing” to continue in the program at the risk your day/time being forfeited.

Make-Up Policy/Student Cancellation Policy
Students will be permitted to schedule 4 make up sessions during the membership if space permits and must be pre-arranged. Please contact your tutor at Beaches Reading Clinic to confirm space for a make-up session and schedule. Single absences will not be added to the end of a membership.

Session Cancellation Policy: Classes/sessions may be cancelled due to holidays, inclement weather and other reasons. Student Memberships may schedule a make-up or opt to add the session to the end of the membership. Please contact Beaches Reading Clinic to arrange during the membership.

Refund Policy for Members: 30 days notice is required to withdraw from your tutoring contract. The account will be fully refunded and/or cancelled after the 30 days are up. Registration fees cannot be refunded. Please submit withdrawal from your tutoring contract in writing to monica_hough@beachesreadingclinic.ca

COVID 19 NOTICE

Beaches Reading Clinic and its tutors adhere to all national and jurisdictional guidelines related to COVID-19. This may include wearing a mask, frequently washing hands, physically distancing from other people in the Clinmic, etc. I understand and acknowledge that my participation in, in person tutoring sessions where there could be close contact between others and tutors increases the potential for contracting or spreading COVID-19. By participating in, in person tutoring sessions I agree to assume these health risks and release Beaches Reading Clinic, its tutors, staff and designates from any and all liability related to COVID-19.
