I agree and/or give permission for my child to participate in all program activities, including chess, chess variants, or other strategy games. I understand that Chess Performance reserves the right to change program activities or instructors and cancel programs, should Chess Performance decide in its sole judgment that it is necessary and appropriate to do so.

I agree to pay any and all registration fees or tuitions prior the the commencement of the activity. If I choose to withdraw from the program after its commencement but prior to its conclusion, I understand that Chess Performance is not obligated to issue any refund. All refunds after the commencement of the activity are issued at Chess Performance's sole discretion.

I understand that all activities designated to be held online have technological requirements, and I agree that I have access to such technologies necessary to effectuate my household's participation in those activities. Chess Performance will not provide any technology required for participants to attend online activities.

With respect to online group lessons, I am aware that Chess Performance’s policy for attendance is one registration per household. If one participant from a household registers for the online group lesson, the entire household may attend if they wish, using the login information that will be provided for each online lesson.

I have informed Chess Performance of any limitations to my and/or my child’s participation and agree to abide by Chess Performance’s sole judgment as to whether I and/or my child can be accommodated in the program. I understand that failing to disclose any physical, emotional, or behavioral needs or conditions may result in the child’s dismissal from the program without refund. Chess Performance reserves the right in its sole judgment to dismiss without refund any child whose behavior interferes with the rights and safety of others or consistently disrupts group dynamics or activities.

I agree to comply by the program's established pick-up and/or drop-off procedures and make all other parents, guardians, or other responsible persons aware of said procedures. I understand that the failure to pick-up or drop-off my child in a timely manner may result in the child's dismissal from the program without refund. Chess Performance reserves the right in its sole judgment to dismiss without refund any child whose parent, guardian, or other responsible person fails to comply with pick-up and drop-off procedures.

I, individually and on behalf of my minor child (or children) and our respective heirs, successors, assigns, and personal representatives, hereby release, acquit, and forever discharge Chess Performance, LLC and their instructors, aides, students, parent volunteers, agents, members, officers, and representatives (in their official and individual capacities) from any and all liability whatsoever for any and all damages, losses, or injuries, including death, to persons or property, or both, including but not limited to any claims, demands, actions, causes of action, damages, costs, expenses, and attorney's fees, which may arise out of, during, or in connection with my or my child's (or children's) participation in the aforementioned program, including but not limited to any damages, losses, or injuries to persons or property, or both, which may be sustained or suffered by me, my child, or any person in connection with my or my child's (or children's) association with, or participation in, the program or any other event sponsored by Chess Performance, or arising out of the same.

I agree that this agreement is intended to be as broad and inclusive as permitted by the laws of the Commonwealth of Kentucky and if any portion hereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
