ASSUMPTION AND ACKNOWLEDGMENT OF RISKS FOR MINORS
Re: use of premises and equipment of the club named Langley Gymnastics Foundation.
To: the above club, operating as Langley Gymnastics Foundation (referred to in this document as the club named) and
Gymnastics BC, and the directors, officers, employees, representatives, officials, landlord and agents of both organizations (collectively referred to in this document as the "agents") .
I have read the guidelines and rules issued for the use of the club named's premises and equipment, which I understand, and I agree to be bound by them. I further agree to acknowledge that:

The rules and guidelines governing the use of the premises and equipment are solely for that purpose, that is, for the use of gymnastics activities, and it remains my sole responsibility to act and govern myself in such a manner as to be responsible for my own safety;
I am aware of the risks inherent in participating in gymnastics activities and the use of gymnastics premises, facilities and equipment and I assume the risks and waive notice of all conditions, dangers or otherwise relating to or arising out of such use.

ADULT RELEASE AND INDEMNITY

Re: use of premises and equipment of Langley Gymnastics Foundation.
To: the above club, operating as Langley Gymnastics Foundation (referred to in this document as “the club named ") and Gymnastics BC, and the directors, officers, employees, representatives, officials, landlord and agents of both organizations (collectively referred to in this document as the "agents").

I have read the guidelines and rules issued for the use of the club named’s premises and equipment, which I understand, and I agree to be bound by them. In consideration of your acceptance of my being permitted to use the premises and equipment and/or any activity associated therewith, I agree to RELEASE, SAVE HARMLESS AND INDEMNIFY the club named and/or its Agents from and against all claims, actions, costs and expenses and demands in respect to death, injury, loss or damage to my person or property wheresoever and howsoever caused, arising out of, or in connection with my use of the premises and equipment notwithstanding that the same may have been contributed to or occasioned by any act or failure to act, including, without limitation, negligence, of the club named and/or any one or more of its agents. I further agree and acknowledge that:

I further agree to HOLD HARMLESS AND INDEMNIFY the club named and its agents from any and all actions, claims, demands, losses, judgments or costs of any nature to any third party resulting from my use of the premises and equipment herein and I agree not to make any claims or take any proceedings against any other person, society, corporation or other legal entity who might claim contribution or indemnity from the club named and/or its Agents in respect of matters which are the subject of this release.

I agree that this Release shall bind my heirs, executors, administrators and assigns. I confirm that I am the full age of eighteen years and I have read this Release and understand it.

COMPANION RELEASE & INDEMNITY AGREEMENT
This waiver only applies to activities at Langley Gymnastics that require a companion (ie parent, guardian, support worker) to enter the gymnastics facility to assist the participant with gymnastics activities. As a precaution, we require everyone to agree to this waiver.

I have read the guidelines and rules issued for the use of Langley Gymnastics Foundation’s premises and equipment, which I understand, and I agree to be bound by them. In consideration of your acceptance of my being permitted to use the premises and equipment and/or any activity associated therewith, I agree to RELEASE, SAVE HARMLESS AND INDEMNIFY LGF and/or its agents from and against all claims, actions, costs and expenses and demands in respect to death, injury, loss or damage to my person or property wheresoever and howsoever caused, arising out of, or in connection with my use of the premises and equipment notwithstanding that the same may have been contributed to or occasioned by any act or failure to act, including, without limitation, negligence, of Langley Gymnastics Foundation and or anyone or more of its agents. I further agree and acknowledge that:

I am aware of the risks inherent in helping my child with gymnastics activities and the use of gymnastics premises, facilities and equipment and I assume the risks and waive notice of all conditions, dangers or otherwise relating to or arising out of such use. I am aware that I am not participating in any gymnastics activity but rather helping my child with his/her gymnastics activity.

I further agree to HOLD HARMLESS AND INDEMNIFY Langley Gymnastics Foundation and its agents from any and all actions, claims, demands, losses, judgements or costs of any nature to any third party resulting from my use of the premises and equipment herein and I agree not to make any claims or take any proceedings against any other person, society, corporation or other legal entity who might claim contribution or indemnity from LGF and/or its agents in respect of matters which are the subject of this Release.

I have read the assumption and acknowledgement of risks for minors/adult release and indemnity statements in this form and agree with the terms.

GYM RULES:

Always think: “SAFETY FIRST!”
Always listen to the coaches and follow their instructions
Treat all coaches, athletes, and equipment with respect
Do not use the gym or equipment without coach supervision and permission
Ask your coach’s permission before getting on equipment or attempting new skills
Follow all equipment and safety rules
Understand the risk of the activity
Be prepared to participate (wear proper attire, always warm up first)
Control your body and actions
Be cautious and aware of your surroundings in the gym
No abusive, threatening or inappropriate language or behaviour
No horse play/fighting
No outside footwear on the gym floor
Those not participating in a program must stay off the gymnastics floor
No photos/videos
No food or drink on the gym floor

TRAMPOLINE RULES

Birthdays, Drop Ins, and Field Trips are NOT allowed on the trampolines. Trampolines are only allowed in structured programs, and with a coach’s permission
One at a time
Perform within your limitations
Bounce in the middle
Bounce with control
Control before height
No inversions (flips) without a coach’s permission
Direct coach supervision required at all times
When getting off the trampoline, stop your bounce, then get off slowly

IF YOU ARE NOT FOLLOWING THESE RULES, YOU WILL BE ASKED TO LEAVE THE FACILITY.

CONSENT FOR USE OF LIKENESS AND INFORMATION
I grant to Gymnastics BC the right to use, without payment of any fee or charge, any photograph, video tape or other visual media of myself for the purpose of furthering Gymnastics BC objectives including but not limited to use for media, inclusion in Gymnastics BC and/or the club‘s publications and website(s), advertising. I further agree that information gathered on formal Gymnastics BC biography forms may be used for the purpose of furthering Gymnastics BC objectives including but not limited to use for media, inclusion in Gymnastics BC publications and website, advertising
