The Kelly Park Sport Association (KPSA) utilizes the Town website, its own Sport.ca website, email and social media to

provide information to the public about our association. From time to time, the Association/Program may post

information regarding team rosters, team accomplishments, team photos, and game action photos that may contain my

child’s name or image. I, the undersigned, agree that by signing my child up for the program I agree to permit my child to

participate in such a publication. I also agree to receive updates from the KPSA through its email communication.

The KPSA DOES NOT accept any responsibility for injury, loss, damage, or accident, either to person or to property

incurred by anyone during the operation of KPSA programs regardless of facility. The undersigned parent or guardian

hereby agrees to release indemnify and save harmless the above mentioned, or any of the against any and all loss, costs,

expenses, claims, demands, and suits whatsoever on account or in respect of any such injury, loss, damage, or accident.

Registration
