Marian Recreation Gymnastics Registration

Online class REGISTRATIONS WITHOUT PAYMENT WILL BE AUTOMATICALLY DELETED AFTER 48 HOURS. Payment for a spot in a class will remove an unpaid spot, in order of date of registration. However, log-in setup and all personal information can be entered and saved prior to finishing the registration process and paying in person.

Important Reminders (please read carefully):

All classes are subject to change based on registration and coach availability
Make-up classes are only available in the event that Marian Gymnastics has had to cancel a regularly scheduled class. There will be no make-up classes or refunds for cancellations that are due to circumstances not in the club’s control.
Full refunds will be available for withdrawal within 1 week prior to class start date.
Any withdrawal within the first 2 weeks of class will receive a 50% refund.
There will be no refund available after the 2nd class.
Refunds for class cancellations will be subject to a $ 25 administrative fee.
Multiple class registrations that result in a refund will be subject to a $ 50 administrative fee.
There is a $25.00 charge on all returned cheques.
- Family Discounts: 5% is taken off cheapest registration. REGISTRATIONS MUST BE ON THE SAME INVOICE.
- Multiple Registrations, by same participant: 5% taken off cheapest registration. REGISTRATIONS MUST BE ON THE SAME INVOICE.

Trampoline Limitatioin:

“Effective April 1, 2019 the insurance carrier for Sask Sport Inc. and Gymnastics Saskatchewan will be implementing limitations on liability coverage for activities and programs involving trampolines and trampoline related equipment. This is a decision the insurer has made nationally after much consideration at their executive level.

All Sport/Markel has made a significant change to what they are willing to offer for insurance coverage for gymnastics across the country, and specifically for our members.

There will be N O liability coverage available for clubs and any participants/members using any type of trampoline equipment during recreational, drop-in, birthday party programs, for casual users, during rentals, and for any outside user group programs taking part in these programs in your gym club.”

As of April 1, 2019 Marian Recreational Gymnastics is required to remove the trampoline, the fast track, and mini-trampolines from rotations normally set up for our school groups, birthday parties, etc. We will replace these bouncy stations with alternative activities and the foam pits will still be available for use.

We are working with Gymnastics Saskatchewan for other insurance so that we may have trampoline equipment back for our outside user groups.

a) Name, address, phone number, and date of birth for the purpose of registering the club members with Gymnastics Saskatchewan and Gymnastics Canada .

b) Name, address, photos, competition results, phone number, cell phone number, fax number and e-mail address for the purpose of communicating about programs, events, activities and social media.

c) Date of birth to determine eligibility, age group and appropriate level of competition.

d) Child Abuse Registry Checks and related personal reference information of coaches for the purpose of implementing Gymnastics Saskatchewan’s screening program.

e) Personal health information including provincial health card numbers, allergies, emergency contact and past medical history for use in the case of medical emergency.

f) Name, address, phone number, cell phone number, fax number and e-mail address for the purpose of providing information to Gymnastics Saskatchewan and Gymnastics Canada for insurance coverage, managing insurance claims and conducting insurance investigations.

RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT

FOR PARTICIPANTS UNDER AGE 18 YEARS OF AGE
AT TIME OF REGISTRATION
In consideration of participation in programs in the Marian Gymnastics Club and with Maximum Gymnastics, their related events and activities, the undersigned parent and guardian of the participant acknowledges, agrees and covenants:

1) The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and, while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and

2) The risks of injury, foreseeable and unforeseeable, are knowingly and voluntarily assumed; and

3) There will be compliance with the stated and customary terms and conditions of participation. If, however, any unusual hazard is observed by me during participation, participation will cease and such unusual hazard shall be brought to the attention of the nearest coach, club or faculty representative immediately; and

4) I, on behalf of myself and the participant, hereby release, waive, discharge and agree to save harmless the Marian Gymnastics Club and Maximum Gymnastics Club, their officers, agents, employees, other participants, sponsoring agencies, sponsors, advertisers, sub-lessors and other parties, who may conduct programs in the facility and conduct events, with respect to any injury, disability, death, loss or damage arising through participation, and whether arising from the negligence or any other cause.

5) I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, AND I FULLY UNDERSTAND ITS TERMS, AND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS ON MY BEHALF, AND ON BEHALF OF THE PARTICIPANT, BY SIGNING IT, AND I AM SIGNING IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
