Release of Liability
I (the registrant, or in the case of a minor, the parent/guardian) agree that I (or the registrant) will abide
by the rules of ST JUDE’S FC, its member organizations and afiliates. Recognizing the possibility of
physical injury associated with soccer and in consideration for the individual organzation accepting
the registrant for its programs and activities, I hereby release, discharge, and/or indemnify ST JUDE’S
FC, its affiliated organizations and sponsors, their employees and associated personnel, including the
owners of gymnasiums and facilities utilized for the Programs, against any claim by or on behalf of the
registrant as a resultof the registrant's participation in the Programs and/or being transported to or
from the same, which transportation I authorize.

Privacy
I understand that any personal information collected on ST JUDE’S FC forms is collected and intended
to be used to enable ST JUDE’S FC to deliver its programs to its members. ST JUDE’S FC is a non-profit
organization. My personal information will be kept secure and will not be shared with anyone other
than those individuals charged with administering the ST JUDE’S FC programs or in cases where
disclosure is required to partlcipate in a ST JUDE’S FC sanctioned event. If ST JUDE’S FC wishes to share
any of my personal information with a third party for any reason, my prior consent will be sought.

Medical
I (or in the case of a minor, as the Parent or Legal Guardian of the named player) hereby give
consent for emergency medical care provided by an athletic trainer, coach, team manager, emergency
medical technician, nurse, medical treatment facility, and/or licensed Doctor of Medicine or Doctor of
Dentistry. This care may be given under whatever conditions are necessary to preserve the life, limb, or
well-being of my dependent.

Release of Information
I give my full permission to ST JUDE’S FC to use my (or that of the registrant) name, likeness, photographs,
videotapes or other recordings of me (and the registrant) that are made during my participation
in ST JUDE’S FC events, for promotional purposes.

Player Movement
I acknowledge that by joining a Program, I understand that I am entering into an agreement with a ST
JUDE’S FC member organization (the 'organization'). As part of this agreement, I understand that I (or
the registrant) shall not participate in any other soccer activity without consent of the organization. I
understand that I (or the registrant) cannot participate in any on-field activities, or sign a registration
with another ST JUDE’S FC member without a formal release from the organization.
