VALLEY GYMNASTICS WAIVER AND RELEASE FORM 2020

I do hereby acknowledge that I (or my child) intend to participate in one or more athletic endeavours while a member of Valley Gymnastics and I am doing so on my/his/her own free will.

I acknowledge that the coaches/advisors will behave prudently in the instruction of gymnastics skills in an effort to avoid injuries and/or accidents from occurring. I realize that participating in athletic endeavours entail the risk of injury to the participants. I accept this risk regardless of the nature of the injury and/or athletic endeavors in which I/my child will participate.

I acknowledge that the term "athletic endeavours" as used herein includes ANY gymnastics activity including but not limited to floor drills, beam use, bar use, air track use and gymnastics of any kind.

I also waive and absolve Valley Gymnastics, the coaches, advisors, sponsors, parent volunteers or other volunteers, the staff and/or volunteers of any venues in which we practice and perform, or any more of them or their executors, administrators, heirs, next of kin, successors or assigns, of and from any and/or all liability and responsibility for injuries, sickness, accidents, and/or natural occurrences during participation by myself/my child in camps, clinics, private coaching, classes, competitions and any other gymnastics activity.

I understand each participant is responsible for his or her own personal health, medical, dental, chiropractic, and accident insurance coverage. MSI coverage and a recent medical are strongly urged.

I, my heirs, executors and/or administration, intending to be legally bound, do hereby waive, release and forever discharge any and all rights for claims for damage which I/my child may have or may hereafter accrue to myself/my child against Valley Gymnastics, the coaches, advisors, sponsors, parent volunteers or other volunteers, the staff and/or other volunteers of any of the venues in which we practice or perform, for any damages which may be sustained or suffered by myself, my child and/or my family in connection with my association with or participation in, or arising out of travel and/or return to any Valley Gymnastics related site or activity affiliated with Valley Gymnastics.

PUBLICITY WAIVER AND RELEASE

I/my child will be participating as a member of Valley Gymnastics. I/my child agree to allow any photos, videos, and/or audio recordings taken of myself at any Gymnastics event to be used by Valley Gymnastics for promotional or educational purposes. These mediums include but are not limited to our website, newspaper articles, and press releases.

I have thoroughly reviewed this package and agree to all of the rules. I am prepared to make the financial commitment necessary to this program.

FINANCIAL COMMITMENT

Currently, our 6 and 12 week gymnastics sessions must be paid in full on or before the start date for the session.

Accounts not paid within terms are subject to a $10 per week late fee.

Athletes will be asked to sit out of classes for any payment over 1 week late.

Athletes with outstanding balances from previous sessions must be paid up-to-date prior to the start of a new session, or will not be permitted to start the new session.

All NSF cheques will receive a $40 service fee from Valley Gymnastics (late fees will also apply).

QUITTUNG/REFUNDS

No refunds or credits will be processed for classes attended by the registered athlete.

An athlete is considered registered until written notification is received by the director (mail or email).

Refunds/credits will not be given for the time the athlete is considered registered (even if they have not been coming to classes).

For any camps or additional classes, we will issue a full refund (MINUS ADMIN FEE) if we receive 7+ days’ notice of cancellation of registration. Any less than 7 days’ notice, a refund will not be issued.

Absolutely no refunds after we have reached the halfway mark of a session (ex. once week 6 is reached in a 12 week session).

All refunds will be minus an admin fee of $25 per transaction.

FINANCIAL AGREEMENT

I have read and fully understand my Financial Commitment to Valley Gymnastics as outlined in this policy document. I understand my commitment is for the session that I am currently registering for (within 2020-2021). I understand that I forfeit money paid if I leave or if I am asked to leave the program. I understand I must pay full fees if I leave, or am asked to leave the program. I understand there are no refunds after the halfway benchmark of my session has been reached. I understand that my child may not participate in or register for classes if payments are not up to date. I understand that any refunds will be minus the Admin Fee.

CLOSING AGREEMENT STATEMENT

After carefully reviewing the above information, I agree with and fully understand everything outlined in this policy document. If registering online, by acknowledging that I have read this document I am agreeing to all of the above. If received in person, by signing below I am agreeing to all of the above.
