En m'inscrivant à ce programme, je confirme que ma fille a subi un examen médical au cours des 12 derniers mois et n'a aucune restriction physique ou autre pour suivre un des programmes de gymnastique rythmique. Je connais et j’accepte les règlements suivants:

1) Toutes les gymnastes doivent porter l'uniforme de sport : leggings ou shorts noirs, t-shirt noir, chaussettes blanches ; cheveux attachés.

2) Les parents ne sont pas autorisés à entrer dans le gymnase et doivent attendre à l'extérieur jusqu'à la fin du cours.

3) Les gymnastes doivent arriver 10 minutes avant le début du cours.
Les parents aident à dérouler / enrouler les tapis.

4) J'accepte de respecter les politiques, règles et règlements du Questo's Club.

By registering to this program, I confirm that my daughter(s) has had a medical examination within the last 12 months and has no physical or other restrictions to follow rhythmic gymnastics programs. I am aware of and accept the following regulations:

1) All gymnasts must wear the sport uniform: black leggings or shorts, black t-shirt, white socks; hair tied back.

2) Parents are not allowed inside the gym and should wait outside until the class ends.

3) Gymnasts have to arrive 10 minutes before the class starts.
Parents help to unroll / roll training carpets.

4) I accept to abide by Questo’s Club policies, rules and regulations.
