Child Registration Agreement (Adult Registration Agreement Below)

I, the parent/guardian of the above named child, hereby give my approval to his/her participation in any and all activities of the Varsity Performance Karate.I do further hereby release, absolve, indemnify and hold blameless the club, the sponsors, the coaches, coordinators and supervisors of Varsity Performance Karate and all members or affiliated organizations. I also consent to the Varsity Performance Karate collecting, using and disclosing the member registration information for all karate participation registration requirements to authorized individuals within the organization and to affiliated karate organizations as required. I give consent for my child to be photographed and video recorded during the program and release use of the images and videos for Varsity Performance Karate promotional purposes, including marketing, technical training, social media and overall online and corporate communications. I acknowledge that I have read and understood the information on this form and that the above named child shall not be considered registered until such time as all required fees, deposits and waivers have been signed and received by the Varsity Performance Karate.

By completing this registration, I agree to complete Varsity Performance Karate Registration Policy.

Should I want to opt-out of any photos or videos to be used/released for Varsity Performance Karate, please contact the club administration, and an alternative registration agreement will be provided. Varsity Performance Karate will not use any photos or videos of you or your child without your consent first.
