Rollerpony Summer camp Terms & Conditions

By Purchasing this activity(activities) I am confirming that I have read & understood the Terms & conditions, Waiver and Liability of Rollerpony’s Summer Camp.

LATE PICK UP POLICY: If your child is not picked up by 5:30 pm, we will need to charge you an additional late fee of $10 for up to and including the first 15 minutes, and $10 for every 15 minutes thereafter.

In consideration of acceptance of this registration by Rollerpony, I hereby agree:
1. That I agree to allow my child to participate in the full camp program and all activities.
2. That the Directors reserve the right to dismiss any Camper for non-compliance with camp policies and/or when it is deemed by the Camp Director to be in the best interests of the child and/or the Camp. that parents shall reimburse the camp for any intentional damage or defacement of camp property by the camper.
3. Rollerpony is not responsible for any damage or loss of personal possessions while at camp or while participating in any camp activity. Campers should not bring valuables to camp.
4. To release, waive, absolve, and agree to indemnify, and save harmless Rollerpony and its Directors, Officers, Agents, and Employees, and any related company from any and all claims and/or liability for any accident, injury, loss, or sickness to my child arising from participation in any camp activities.

5. That your child's attendance at Rollerpony and your relationship with Rollerpony, its Directors, Officers, Employees, Medical Staff and Agents shall be governed by the laws of the Province of Ontario and you shall submit to the exclusive jurisdiction of the courts of the Province of Ontario in that regard.
6. If for any reason my child requires medical attention or special medication beyond that furnished by Camp, I agree to be responsible for any expenses incurred.
7. THAT IN CASE OF MEDICAL EMERGENCY, I hereby give permission to the physician and/or Camp Officials to hospitalize, secure proper treatment for, and to order injection, anaesthesia or surgery for my child, as named above. Every effort will be made to contact the parent(s), /legal guardian, beforehand.
8. That you will keep Rollerpony updated on any new medical conditions/information not listed on this application prior to camp.
9. I agree to update the camp of any prior camp or school issues (suspension, discipline) or social, medical, emotional, or psychological issue or any other relevant matter prior to camp that could affect my child or any other child's enjoyment. By purchasing this activity I confirm I am a legal guardian of my child(ren) and custodial parent(s), and that all legal guardian(s) are in agreement and are aware their child is attending Rollerpony summer camp and that whoever executes this contract has the legal authority to be able to make the decision to send their child to camp on behalf of the legal guardian(s). Legal guardian(s) agree to read the terms of this contract and agree that unless they contact us in writing they both agree to the terms within. I hereby certify that I am the adult parent or guardian of the named minor child under the age of eighteen years, and I consent to his/her participation in art enrichment programs and activities at Rollerpony located at 75 Westmore Drive in Etobicoke Ontario and any other location venue hosted by Rollerpony. I am fully aware that during camp programming (applies if registering the minor child for Summer Camp), camp participants will sometimes be outside for portions of the camp for play activities dependent on weather. The above also applies to all other services offered at Rollerpony involving minor children under the age of 18.

I confirm that my child(ren) is capable of participating safely in the full camp program unless I/We advise you in writing and the camp confirms receipt of such information. I further acknowledge that attendance and/or participation at Camp involves risk and hazards incidental thereto, all of which are assumed by me. I hereby waive, release, absolve and agree to indemnify and save harmless The Imagination Studio and their respective officers, directors, employees and agents of any and all liability arising from my child's attendance and/or participation of the Camp program (unless solely as a result of the Camp's willful neglect or willful default). I understand and acknowledge that I am fully aware of and assume the risks (including but not limited to the risk of serious bodily injury, allergic reactions, property loss or damage) of my minor child's participation in art enrichment programs and activities at Rollerpony. I recognize my responsibility to ensure that my minor child participates only in those activities for which he/she has the required skills, qualifications, and physical conditioning. I understand that Rollerpony shall have no responsibility to pay for medical treatment and related costs if said minor child is injured. Knowing the risks described above, I agree, personally and on behalf of my minor child with whom I am registering, to assume all the risks and responsibilities surrounding my minor child’s use of art supplies, art equipment and experiences during field trips or otherwise at the studio. To the fullest extent allowed by law, I hold harmless and agree to indemnify Rollerpony, volunteers, employees and agents, from and against any present or future claim, cause of action, loss or liability for injury to person or property, which said minor child may suffer or for which said minor child may be liable to any other person, related to said minor child’s participation in recreational activities at Rollerpony, resulting from any cause whatsoever, and regardless of fault. I am at least eighteen years of age and have carefully read and freely signed this Liability Waiver and Release Form (Minor Child). I understand and agree that no oral or written representations can or will alter the contents of this document.
